My child _________________________ has permission to attend the field trip to Seattle Childrens Theater on Tuesday, December 1, 2009 with the preschool class. I understand that transportation will be by private car or school van and that I need to provide a car seat for my child to be left at school that day (labeled with my child’s name on it).

__________ I can drive and also bring ________ preschoolers OTHER than my own, along with _________ adults in my car/van.
__________ I am unable to drive this time, and my child will need transportation in the school van or private car, and I will provide a labeled booster seat.
__________ I would like to chaperone on this field trip, and I will provide my child with a labeled booster seat.
